You will see the following error message on the ERRORS page in H&R Block’s 2016 tax software if, while completing Schedule 8: Contributions to the CPP or QPP, you entered an invalid date in the field labelled If you restarted your CPP contributions in 2016, enter the date you completed the CPT30: Election to stop contributing to the Canada Pension Plan, or revocation of a prior election form:

The date you completed the CPT30 form to restart your CPP contributions must be between 2016-12-31 and 2017-12-31. Please correct the date you entered. [error code: 349]

How do I fix this?

To correct the error, enter the date on which you completed form CPT30 in order to restart your CPP contributions. The date you enter into the tax software must be between 2016-12-31 and 2017-12-31.
